Keynote Speaker - Lisa Ferentz

Lisa Ferentz, LCSW-C, has been in private practice for over 35 years and is a nationally and internationally recognized expert in the treatment of trauma.
She was an adjunct Faculty member at several Universities, is a trainer and consultant to clinicians and mental health agencies throughout the US, Canada, the UK, and Ireland, and is the Founder of The Frentz Institute, now in its eleventh year of providing continuing education to mental health professionals.
Lisa is the author of  Treating Self-Destructive Behaviors in Trauma Survivors: A Clinician’s Guide now in its second edition,  Letting Go of Self-Destructive Behaviors: A Workbook of Hope and Healing, and Finding Your Ruby Slippers: Life Lessons From The Therapist's Couch.
In 2009 Lisa was voted the “Social Worker of Year” by the Maryland Society for Clinical Social Work.

EARLY REGISTRATION DEADLINE

Registration and payment must be postmarked no later than September 28, 2018 to receive the early-bird rate

 

CONTINUING EDUCATION

NASW, Iowa Chapter, has determined that this program meets the criteria of the Iowa Board of Social Work Examiners, Iowa Board of Certification, and the Iowa Board of Behavioral Sciences Examiners for 14.1 hours of continuing education for social workers, marriage and family therapists, and mental health counselors. 6.9 hours of CEUs available on Thursday, October 18, and 7.2 hours on Friday, October 19.

SPECIAL ACCOMMODATIONS

A registrant who requires ADA accommodations to attend or participate in the Annual Symposium should include a written request for accommodation with the completed Registration Form and fee payment by September 14, 2018 The written request must include the specific accommodation required. Every effort will be made to accommodate a special accommodation request received after September 14, 2018 but we cannot guarantee the accommodation.

IMPORTANT REGISTRATION INFORMATION

  • All mailed registration forms must be filled out completely, legibly, and be accompanied by payment or purchase order.
  • Registrations to be paid by credit card shall utilize the online registration process.
  • Confirmation email or postcard will be mailed within five days of receipt of registration.

Reservations should be made directly with Holiday Inn DSM NW, 4800 Merle Hay Rd., Urbandale, IA, 50322, at 800/465-4329 or 515/278-4755 by September 26, 2018 to receive the guaranteed rate. Please indicate you are attending the group registration code 017 to qualify for the special room rate. 

AWARDS LUNCHEON

The awards reception, Friday, October 19, is open to all. Registration to the luncheon is included with paid registration fee. One may choose to attend the awards luncheon only by completing the registration form and including payment in accordance with conference deadlines.

PAYMENT METHODS

Registration fees may be paid by cash, check, purchase order, or credit card (American Express, Discover, Mastercard, Visa). All credit cards will be processed via online registration process. If you are paying by purchase order, please provide a copy of the completed PO with your completed registration form.

Dress in layers – temperature in the rooms may vary.
